Was erwartet dich?
- Du verantwortest die professionelle Überleitung von Patient:innen aus der Klinik in ihr häusliches Umfeld und stellst so eine lückenlose Versorgung sicher.
- In enger Abstimmung mit Ärzt:innen und Klinikpersonal ermittelst du den individuellen Bedarf und passt spezifische Medizinprodukte an, insbesondere im Bereich der Wund- und Stomaversorgung.
- Als zentrale Ansprechperson schulst du Patient:innen, Angehörige sowie Pflegekräfte im sicheren Umgang mit den Hilfsmitteln, um die Therapieziele zu erreichen.
- Du betreust deinen Patient:innenstamm eigenverantwortlich weiter, passt die Versorgung bei Bedarf an und sicherst so eine hohe Behandlungsqualität.
- Du baust dein regionales Netzwerk aktiv aus, indem du neue Partnerschaften mit Kliniken sowie Pflegeeinrichtungen knüpfst und unser Unternehmen auf Fachveranstaltungen repräsentierst.

Was solltest du mitbringen?
- Du hast eine abgeschlossene Ausbildung als examinierte:r Gesundheits- und Krankenpfleger:in oder Altenpfleger:in.
- Idealerweise bringst du bereits Erfahrung in der i.V.-Therapie und eine Qualifikation als Wundmanager:in (ICW) oder Stomatherapeut:in mit.
- Als kommunikatives und organisationstarkes Talent arbeitest du gerne im Team, übernimmst aber auch selbstständig Verantwortung.
- Du besitzt einen Führerschein der Klasse B, um deine Patient:innen im Außendienst bestens betreuen zu können.
- Deine sehr guten Deutschkenntnisse in Wort und Schrift runden dein Profil ab.

What do I have to consider if I am not from Germany?
Please make sure to provide all necessary documents within your Workwise profile. It should include an EU work-permit (if you have no EU citizenship) and a CV at least.
