Regelmäßige ärztliche Versorgung im häuslichen Umfeld im Rahmen der SAPV.
Was erwartet Sie?
- Sie übernehmen die ärztliche Versorgung von schwerstkranken Menschen in ihrem Zuhause, steuern die Symptomkontrolle und entwickeln individuelle Therapiepläne im Rahmen der SAPV.
- Sie gestalten aktiv den Aufbau unserer neuen SAPV-Einrichtung mit und prägen so von Beginn an die Versorgungsqualität und die inhaltlichen Strukturen.
- Als zentraler medizinischer Ankerpunkt arbeiten Sie eng mit unserem multiprofessionellen Team zusammen und bringen Ihre Expertise in regelmäßigen Fall- und Teambesprechungen ein.
- Sie pflegen ein starkes Netzwerk zu Hausärzt:innen, Kliniken sowie weiteren Kooperationspartner:innen und sichern so eine nahtlose Versorgungskette.
- Sie gewährleisten durch eine sorgfältige Dokumentation und die Teilnahme an Rufbereitschaften die lückenlose Betreuung Ihrer Patient:innen nach SAPV-Richtlinien.

Was sollten Sie mitbringen?
- Sie sind approbierter Arzt (m/w/d) mit der Zusatzbezeichnung Palliativmedizin.
- Sie bringen nachweisbare Erfahrung in der Betreuung schwerstkranker und sterbender Menschen mit.
- Ihre Arbeitsweise ist geprägt von Empathie, Teamgeist und hoher sozialer Kompetenz im Umgang mit Patient:innen und deren Angehörigen.
- Sie haben Lust darauf, den Aufbau unserer neuen Einrichtung aktiv mitzugestalten und eigene Ideen einzubringen.
- Für die ambulante Versorgung besitzen Sie einen Führerschein der Klasse B.
- Sie sprechen fließend Deutsch (C1-Niveau), um eine einfühlsame und klare Kommunikation zu gewährleisten.

What do I have to consider if I am not from Germany?
Please make sure to provide all necessary documents within your Workwise profile. It should include an EU work-permit (if you have no EU citizenship) and a CV at least. Depending on the position you are applying to, you could also be asked for a certificate of enrollment, a transcript of records or a language certificate.
