Was erwartet Sie?
- Sie übernehmen die eigenständige palliative Versorgung unserer Patient:innen im ambulanten Dienst und schaffen eine würdevolle Atmosphäre in ihrem gewohnten Umfeld.
- Sie erstellen und steuern individuelle Pflegepläne in enger Abstimmung mit unserem interdisziplinären Team aus Ärzt:innen und Therapeut:innen.
- Als zentrale Ansprechperson beraten und begleiten Sie die Angehörigen in dieser herausfordernden Lebensphase.
- Sie sichern die Behandlungsqualität durch eine sorgfältige Pflegedokumentation und bringen Ihre Expertise aktiv in regelmäßige Fallbesprechungen ein.
- Sie gestalten den Aufbau unserer neuen Palliativeinrichtung aktiv mit und bringen Ihre Ideen und Erfahrungen ein, um die Zukunft der Pflege zu prägen.

Was sollten Sie mitbringen?
- Sie bringen eine abgeschlossene Ausbildung als Pflegefachkraft (m/w/d) sowie eine Weiterbildung im Bereich Palliative Care mit.
- Für die ambulante Versorgung besitzen Sie einen Führerschein der Klasse B.
- Ihre Arbeitsweise ist geprägt von Empathie und Respekt im Umgang mit Patient:innen und ihren Angehörigen.
- Als zuverlässiges Teammitglied schätzen Sie die enge Zusammenarbeit im interdisziplinären Team.
- Sie haben Lust darauf, unsere neue Einrichtung aktiv mitzugestalten, und bringen eine hohe Eigenmotivation mit.
- Sie sprechen fließend Deutsch (C1-Niveau), um eine klare und sichere Kommunikation zu gewährleisten.

What do I have to consider if I am not from Germany?
Please make sure to provide all necessary documents within your Workwise profile. It should include an EU work-permit (if you have no EU citizenship) and a CV at least. Depending on the position you are applying to, you could also be asked for a certificate of enrollment, a transcript of records or a language certificate.
